Welcome to our office! For each new patient, please use the link below to fill out your forms before your first appointment. If you are unable to do so, please arrive 15 minutes prior to your appointment to fill out the necessary paperwork.
Your Initial Appointment
Your initial appointment will consist of a comprehensive oral evaluation. Following your exam, you will have a one-on-one consultation with one of our dentists. The dentist will explain your oral health diagnosis. We will then determine a treatment plan that’s specific to your individual needs, wants, budget, and schedule. Please use our Online Health History form to save you time at check-in.
Additional Privacy for Substance Use Disorder (SUD) Treatment Records: Under Federal Law (42 CFR Part 2) , we provide additional protections from records that identify you as have a substance abuse disorder. We will not use or disclose these records for civil, criminal, administrative, or legislative proceedings against you unless we have your specific written consent or a court order that meets federal requirements. You may provide a single written consent that allows us to use and disclose your SUD records for all future treatment, payment, and health care operations. You have the right to revoke this consent at any time in writing, except to the extent that we have already acted in reliance on it. Patients have the right to opt out before using or disclosing their SUD records for fundraising activities. Information shared under the HIPAA Privacy Rule may lose its federal protection and could be suspect to redisclosure by the recipient.
